Mr. Rivera is a 72-year-old patient with end stage COPD who is in the care of Hospice. He has a history of smoking, hypertension, obesity, and type 2 Diabetes. He is on Oxygen 2L per nasal cannula around the clock. His wife and 2 adult children help with his care. Develop a concept map for Mr. Rivera. Grading Criterion:

   The student must list at least 3 risk factors related to the medical diagnoses

  The student must list at least 5 possible/anticipated medications that the client may be on

  The student is to describe a subjective statement that they client may say related to the medical diagnosis

  The student is to list at least 3 objective assessments that would be expected with a client who has the medical diagnosis

  The student is to list at least 5 specific sources of diagnostic data that would relate or be expected to be ordered for the client who has the medical diagnosis.

  Identify at least two possible nursing diagnosis written in a 3-part format based on your subjective and objective criterion you established (3-part = Nursing diagnosis .... related to (R/t) symptoms.... as evidenced by (AEB) your subjective and objective criterion 

  Identify at least 3 nursing interventions that you would do for your client (NO MEDICATIONS)

  Identify at least 3 medical interventions such as medications or treatments

  List at least 3 major teaching points such as teaching about medications, risk factors, medical procedures or anticipated diagnostic procedures.
