Conflict can be defined as a state of disharmony that occurs within a group of people with differing viewpoints, opinions, values, or priorities.

Conflict setting :

I entered into the emergency room for my 12-hour shift in the emergency department and found out that my assignment that day was quite heavy. I was assigned seven patients, including two who were unstable at that time. I noticed that my colleague had a light assignment with only five patients, all of whom were scheduled to be discharged to home during the shift.

I felt frustrated because my patient load exceeded the ER guidelines regarding patient number and acuity. I had earlier noticed that my colleague often receives lighter assignments because she is known to be intimidating and aggressive. I decided to address the issue with the charge nurse who told me she was busy and “it is what it is.” Increasingly frustrated, I decided that it was time to approach the unit manager about this ongoing issue.

Insight

Collaboration is considered the most effective method of conflict management.

The manager could have an informal meeting with both the nurses and the charge nurse where they can discuss ways they can all come to an acceptable solution.

Each party can give input and feel their opinion is important and heard by management. With all involved parties included, each will be more willing to compromise for the overall benefit of everyone on the unit including other staff members and patients. This solution offers the best way for a speedy resolution while also improving patient care and patient outcomes.

Another important aspect could be negotiation.

Negotiation involves controlling reactions during the conflict, seeking to actively listen and understand the other party’s issues, identifying the unmet need, and being able to compromise and collaborate to come to a positive solution.

Anxiety and depression are the most common disorders among old people, later in life depression often hits as one thinks about their past and remember things that used to be, and the prognosis is usually long-term and can be chronic with higher relapses than with younger people (Afshari et al., 2019). The risks are higher for older people to have depression if they are isolated, have a chronic illness, and are lonely, those that have high family support, as well as income stability, often are less of a risk.

When it comes to younger adults there are other factors that can be used to help determine risk, such as biological and psychological factors, this too can be used for older adults as it would be age-related. Younger people can also feel depressed with loneliness, and they are at the most risk.

When comparing depression between elderly and younger people, it seems that depression is mainly caused by the mitigating factors of one’s life. Older adults tend to have symptoms of agitation, hypochondriasis, gastrointestinal issues, reduced guilt, and loss of sexual interest; research studies with elderly and younger people using information from RDC, ICD-9, ICD-10, DSM-III, DSM-III-R or DSM-IV criteria, on average older people often had more severe depression and when it came to psychotic depression both age groups were affected severely, younger people had more guilt (Hegeman et al., 2018).

When research studies used the Duke Depression Evaluation Schedule for the Elderly and Montgomery-Åsberg Depression Rating Scale there was found to be similar phenomenology in depression for those over 60 years of age versus those less than that age, but overall research suggests that older adults are depressed more often than that of their younger counterparts (Hegeman et al., 2018).

When a person drives, they need to be able to react quickly to avoid a situation such as an accident. Someone that has dementia may not be able to react quickly enough which causes accidents where people are hurt or in some cases die. Dementia is a severe neurocognitive disorder it affects about 50 million people globally the most common disorder is Alzheimer’s disease (AD), which is neurodegenerative dementia; cognitive functions that are needed for driving are decreased when a person has dementia, and one must pay attention when driving and when someone has dementia these functions are impaired (Falkenstein et al., 2020).

So anyone that has dementia or AD should not be driving. They should lose their license and their right to drive.

IDENTIFYING DATA

LS is a 25-year-old Caucasian male who works as a lawyer at a top law firm in his state. He came to the clinic accompanied by his romantic partner. They are both worried about his condition since it has been affecting his social life and has caused a change in his professional environment. 

CHIEF COMPLAINT: “I feel confused about how to act.""

HISTORY OF PRESENT ILLNESS

The patient's overall life has been affected due to a limited ability to attain emotional stability. When he became aware of the initial diagnosis, he started getting affected by physical symptoms like pain, shortness of breath, or body weakness. All these indicate that the patient was impacted by a response to psychological distress, mainly discovered when a person is experiencing these symptoms. As LS progressed with his life and taking medication, he was asked to take some medical tests, but they did not always show a medical basis. The previous doctor diagnosed him with a personality disorder because psychological distress and physical symptoms were the main symptoms he displayed. 

PERSONAL HISTORY

LS has had a rough time adjusting his life so that other people would be accommodative to his personality. He often changes moods quickly and can appear to be an impatient person, yet the issue is known to persons close to him. Career advancement has been a common outcome of LS's life since he does not allow his personality issues to affect his sustenance. 

PAST PSYCHIATRIC HISTORY

The patient got diagnosed with a personality disorder when he was 15 years old. It was imperative for him to receive the diagnosis since he had been frequently affected by emotional distress and unmanageable behavior when he joined high school. 

MEDICAL HISTORY

The patient had been prescribed Serotonin reuptake inhibitors which is a type of antidepressant used in patients with personality disorders who get affected by severe physical and psychological symptoms. The medication is an anxiolytic agent who got prescribed to the patient by his physician. LS explained that he researched the medication he prescribed and discovered that patients could become dependent on them. Thus, he managed his medical history by communicating with his nurses since he started taking the medication 3 months ago.
